All students and parents must complete the following paperwork before the student is allowed to tryout/participate in Griffin Sports.  

 

1. Rank One: FISD Athletic Paperwork (Click here)

-All of these forms are online only (no need to print anything).

-You can also sign up on Rank One for schedules/email alerts (changes in games, times, etc.).

2. Griffin Athletic Contract Acknowledgement Form

-Complete the acknowledgment form after reviewing this Griffin Athletic Contract.  You do not need to print anything for this.

3. 2020 Medical History/Physical Form (Click here) (must say 2020 in top right corner!):​

     >Medical History side: Filled out and signed by a parent and student

     >Physical side: Filled out and signed by a doctor

-Be sure to use the updated 2020 Medical History/Physical Form (2020 printed on top right corner!).

-Must be dated after May 1 of the current year.

4. Impact Baseline Concussion Test (Click here)

 - ​Important to note:  This test can be taken on a chromebook or a desktop computer and usually takes anywhere from 30-45 minutes to complete.  Please read all instructions on the linked form above before beginning the test.​

 - you do not need to print anything out
